If it seems like it could be serious, go get someone to confirm if it's accurate or not. Health issues? Ask a doctor. Work issues? Ask someone in charge. Relationship issues? Be direct rather than trying to guess what someone else is feeling. The more you cut through all the crap that's out there (and especially in your mind), the easier it's going to be to disregard things that aren't worth worrying about in the first place.
